G3.1.2 Requirements for Performance Evaluation System

Purpose
To state the requirements for the performance evaluations (also referred to as performance appraisals) utilized by the Indiana University Police Department (IUPD).
General Order
The IUPD follows Performance at IU guidelines which include a documented annual evaluation. Supervisors will complete annual performance appraisals for each of their direct reports. The appraisal will be reviewed and approved by the second-level supervisor prior to being delivered to the employee. Employees will have an opportunity to provide comments and document their goals. Once finalized, a signed copy will be provided to the employee and placed in the employee’s formal personnel file.
Evaluation period (3.1.2 a)
The IUPD follows the Performance at IU timeline which is typically January 1st to December 31st. The performance appraisal will be an evaluation of the employee's performance during this time period.
Explanatory statements (3.1.2 b)
Supervisors will typically provide descriptions or explanatory statements of each skill on which the employee is being evaluated, however, these explanatory statements are required for any skill with a rating of unsatisfactory or outstanding. In the comments section below each skill, supervisors will typically provide detailed notes and exact examples of how the employee met, exceeded, or did not meet the requirements.
Each performance evaluation report is reviewed and confirmed by the rater’s supervisor (3.1.2 c)
After completion of the employee appraisal form and prior to meeting with their direct reports, the first-level supervisor will share each appraisal form with the second-level supervisor for review and approval.
Employee is given an opportunity to sign and provide comments on the performance evaluation (3.1.2 d)
The first-level supervisor will provide each of their direct reports a copy of the reviewed appraisal form at the end of the meeting so the employee has an opportunity to review it and provide comments. Indiana University Human Resources may review the employee’s comment(s), and the university reserves the right to remove or request an edit of the employee's comments before they are finalized.
The employee will sign and return the appraisal form to their supervisor within one business day of the original meeting. Employee signature indicates acknowledgement and receipt of copy only; it does not indicate agreement. If the employee refuses to sign, the supervisor will indicate this on the signature line.
After the employee has had an opportunity to review, provide comments on, and sign the appraisal form, the first-level supervisor will sign the appraisal form and submit it to the second-level supervisor for final review and signature. The final appraisal form will include all comments from the supervisor, the employee, and all signatures. The supervisor will provide a copy of the completed form to the employee for their records. The supervisor will also forward a copy of the completed form to appropriate personnel to ensure it is uploaded in the employee’s formal personnel file.
