Purpose
To state the requirements for preparing a report by the Indiana University Police Department (IUPD) personnel who process a crime or traffic collision scene.
General Order
IUPD personnel who process a crime or traffic collision scene will prepare a written report that documents all tasks performed and evidence collected at the scene.
Report Preparation (14.1.8)
A written report will be prepared by the person who processes a crime or traffic collision scene. This report will serve as an accurate record of events which transpired at the scene and/or in connection with a follow-up investigation and illustrates the chain of custody.
Elements to be included in the report may include, but are not limited to:
- Date and time the request for service was received;
- Date and time of arrival at the scene;
- Location of the incident;
- Victim, witness and suspect names;
- Name of the investigating officer;
- Conversations with individuals at the scene;
- Evidence collection and other actions taken at the scene (photographs, measurements, sketches, fingerprinting, etc.);
- List of physical evidence seized or recovered;
- Disposition of the physical evidence;
- Scene measurement information;
- Sketches and scale diagrams prepared;
- Incident report case number
If no evidence was recovered after processing or if the scene could not be processed due to contamination, personnel will document this in the case report.
If another agency responded and processed the scene, the responding IUPD officer will note this in the case report and document the other agency’s report number.
