Purpose
To provide the Indiana University Police Department (IUPD) with guidance related to reporting use of force incidents.
General Order
Officers will complete a Critical Decision Making (CDM) incident form and a case report in the IUPD Records Management System (RMS) whenever an officer on or off duty:
- discharges a firearm (other than in training or for recreational purposes) (7.1.10 a);
- applies force using lethal or less-lethal weapons (7.1.10 b); or
- applies any level of force, including weaponless force, in order to gain compliance, including restraint beyond compliant handcuffing (7.1.10 c).
The CDM incident form and case report will be completed prior to the end of an officer’s duty shift if the incident does not rise to the level of deadly force.
Following the principles of trauma informed practices, for incidents that do rise to the level of deadly force, the involved officer(s) will complete the CDM incident form and case report in the RMS after at least two sleep cycles to allow for time to accurately recall and document the event. The officer(s) may elect to complete all required paperwork before the two sleep cycles. The supervisor will still be required to complete the initial case report in the RMS prior to the end of their duty shift.
If an officer is injured and unable to complete the CDM incident form within seventy-two (72) hours of the incident, the supervisor will complete the form on behalf of the officer
